Am I a Good Candidate for Thermage? What a Seoul Dermatologist Checks First

Who is a good candidate for Thermage, and who is not?
Thermage suits skin that has lost elasticity rather than skin weighed down by volume. It is generally a reasonable starting point for mild overall laxity, fine lines around the eyes and mouth, and a jawline that has begun to soften without a heavy fat layer. Where sagging is pronounced or fat volume is significant, a combined approach is usually discussed instead.
| What is being treated | Loss of elasticity, not fat volume |
|---|---|
| Usually suits | Mild overall laxity and early jawline softening |
| Also often raised for | Fine lines around the eyes and mouth |
| Less likely on its own | Pronounced sagging or a heavy fat layer |
| How it is decided | In-person assessment by sight and touch |
| Device used at AUVEL Clinic | Thermage FLX |
| Tell the clinic beforehand | Pregnancy, implanted electrical devices, recent facial procedures |
Age is the wrong starting question
Patients almost always open with their age. It is an understandable question and a poor predictor. Two people born in the same year can have very different skin thickness, very different elasticity, and very different reasons for the change they see in the mirror.
The useful question is what kind of change it is. Skin that has become slack behaves differently from skin that is being pulled downward by weight beneath it, and the two respond to different things. Radiofrequency heats the deeper layers so existing collagen contracts and new collagen is gradually laid down over the following weeks — that addresses elasticity. It does not remove volume.
So a face that reads as tired because the skin has loosened is a different case from a face where fat has descended along the jaw, even when the two people are the same age and describe the concern in identical words.
What is actually examined before treatment
At AUVEL Clinic the skin is assessed in person: looked at, and touched. Thickness and elasticity cannot be judged from a photograph, and they are the two properties that decide how much energy the skin should receive and at what depth.
The assessment also establishes where the concern began. A jawline can soften from the cheek downward or from the neck upward, and the two are planned differently even on the same device.
From that, the depth and amount of energy are mapped per area of the face. This matters more than it sounds: a plan built around a fixed number of pulses treats every face as the same face, which is precisely what the examination exists to avoid.
When Thermage is a reasonable place to start
In practice the treatment is most often discussed with people who recognise themselves in one of the descriptions below. These are patterns that make the treatment worth considering, not predictions of how any individual will respond.
- Skin has become mildly slack overall, but nothing has dropped dramatically
- Fine lines around the eyes and mouth have started to hold their shape
- The jawline has begun to blur, without a thick layer of fat beneath it
- You want a treatment that does not leave open wounds or scabbing
- You are able to wait months rather than days for the change to appear
When it usually is not the whole answer
Where the skin has descended significantly, or where the volume beneath it is substantial, tightening the skin alone tends to under-deliver against what the patient is picturing. In those cases the honest conversation is about combining approaches rather than about repeating the same treatment more often.
There is also a timing case. If you are planning around a wedding or a specific date and that date is weeks away, the problem is not suitability but the calendar — the visible change from collagen remodeling does not arrive that quickly.
A consultation that ends with a recommendation not to proceed is a normal outcome, not a wasted appointment.
- Pronounced sagging along the jaw and neck
- A significant fat layer driving the change in contour
- An event in the next few weeks that you want to look different for
What to tell the clinic before you book
Some of what decides suitability is not visible on your face at all. The items below are worth raising when you make contact rather than on the day, because they can change the plan or the timing.
This is not a checklist that clears or disqualifies you by itself. It is what allows the doctor to make that judgment properly in person.
- You are pregnant, or think you may be
- You have a pacemaker or another implanted electrical device
- You have had injections, threads or laser treatment on the same area recently
- You are taking medication for a skin condition, or being treated for one
- You have had an unexpected reaction to a previous aesthetic treatment

Frequently asked questions
Am I too young for Thermage?
Age is not the criterion. What is examined is skin elasticity and thickness, and where the change in contour is coming from. Some people in their thirties are suitable and some in their fifties are better served by a combined approach.
How do I know whether my jawline is sagging skin or fat?
This is difficult to judge from the mirror and is one of the main reasons the assessment is done by sight and touch in person. Skin that has lost elasticity and a descending fat layer can look similar and are planned differently.
Can I have Thermage if I have had fillers or threads?
Tell the clinic what was done and when, before you book. Previous treatment on the same area does not automatically rule anything out, but it can change the plan or the timing, so the doctor needs to know in advance.
What happens if Thermage is not right for me?
The consultation ends with that recommendation. Depending on what is driving the change, a different treatment or a combination may be discussed instead. A consultation that concludes with 'not this' is a normal outcome.
Is Thermage suitable before a wedding?
Only if there is time. The visible change develops over months rather than weeks, so the limiting factor close to an event is the calendar rather than whether your skin suits the treatment.
Can I be assessed remotely before flying to Seoul?
You can describe your concern in English over WhatsApp beforehand, and that is worth doing so the visit is planned around it. The suitability judgment itself is made in person at the clinic, because thickness and elasticity cannot be assessed from a photograph.
